Name Used Without Permission as Director for 501(c)

My name was provided in an application apparently for a 501(c) 3 charity with my permission or notification at all. I found it, coincidentally, when googling my own name. What recourse do I have?

 I would contact http://www.lawhelp.org/ in your area to see what legal recourse you may have. Other than that see a lawyer.
